One of the many fundamental facets of recovery is taking medication as directed. Risks associated with incorrect or missed doses could be detrimental to the patient’s health. Failure to take medication in the right dosage, at the right time, in the right frequency can lead to hospitalizations, the disease worsening, or even death.
There are new medications constantly hitting the market to increase quality of life for patients; however, with these new medications often comes risks. When a patient is taking multiple medications, especially from more than one doctor, it is important to understand how the medications will interact with each other. It can also become difficult to weigh the benefits of medications in the realm of complex diseases.
Serenity Home Health implements a multidimensional coordinating process that ensures the patient receives the correct dosage of medication at the correct time, every day. We strive to ensure the medications are safe to take together and monitor that the side effects do not outweigh the benefits.
Some of our services include:
- Helping to create a medication regimen and schedule
- Educating the patient on proper dosage and side effects
- Ensuring prescriptions are filled
- Reviewing prescriptions prescribed by different doctors to prevent potentially dangerous drug interactions
- Evaluating responses to the medications
- Assessing the effectiveness of pain management
- Communicating with the patient’s doctor regarding the medication’s therapeutic response
Medication education is provided to each patient. This includes information about each medication such as its purpose, dosage, how to take it, how frequently to take it, as well as possible side effects.
